TunnelBear is good and pretty cheap. I emailed them and asked for a discount and they gave me 20% off a yearly subscription. They are Canadian based though.
It is really really simple, and they claim to keep no records at all. Downside, being based in Canada and record keeping, is that it does not allow torrenting.
