Local business Wordpress theme

Any one suggest a local business theme which has following out of box. I am aware that contact form can be added to home page by customizing it a bit , but i am looking for something which is out of box.

following are what i am looking for.

1. Short Contact form on home page and every subpages
2. Phone number and email address on Top Right
3. Provision to add local address on footer
3. Address and Map on contact us page.

2-4 is available in many local lead gen themes but having contact form on home page and all the subpages are finding difficult in many wordpress themes. Even if it is possible , we need to have set page as static homepage. In Joomla it is easily possible but concerned about security.

I had them set it up for me, but its pretty easy to get what you are looking for and cheap. Just had time constraints when I wanted to go with a new theme, which involved moving servers too. You can PM me if you want to see my business site. The keywords I want to rank seem to be based on how frequently I post to the blog, once or twice a week is perfect, and I stopped doing just about everything else.

I set this up for another small business in about 2 hours total.
