I got started with this a couple of days ago - looking forward to see how it plays out. As far as approval with CrakRevenue is concerned, I think I just happened to get pretty lucky.
Now, I'm just working to not look at my stats.

It's not a get quick rich scheme, but worked well for me
I posted my stats, I've had days where I got 5000 UI's and no conversions, and some days I got 100 and made 100$, it just varies a lot :/

I think direct to bank, check, and there's something else. I use checks as the fee was the least.
Yeah it's hard not to look at stats like every 10 mins. But you have to let it go and not worry to much, just keep adding banners and picking the ones that work best for you

Thank you for the sharing this method. I've already been accepted and am waiting for exotic ads to authorize my card. One question: you said you made your own ads. I've had a look at some programs, but do you know the best one that's simple and ideally free?

I've actually been using a free site called fotor for mine, and it works rather well!

The one thing most of you will do and fail is join just this one advertising source the OP mentioned. A smart thing to do and this wont get saturated is do some research for other adult sites that don't have so many advertisers on it. Contact the site owner and ask them how you can advertise on their site. They may be using a advertising company or they may work out a monthly deal with you.
