Yes, sitewide links are still frequently used and they work fine. I've used them a couple of times to diversify link profile anchor texts. To avoid long term risks, when doing so you should pay attention to this:
- The source sitewide linking to you must be relevant to your content and share the same niche.
- Don't use an exact match anchor. You don't want that optimized anchor to have more weight in your link profile than your brand name or URL.
- The more quality and healthy link profile the source has, the less risk you will get.

Sitewide links are still popular and will probably always remain that way. But it is important to make sure they're from relevant niches. Sitewide links from others can easily be looked at as spam. When they are from relevant sites, Google could just assume that they're citing you or numerous other things.

They are risky if used with anchors. It also makes a difference how big the site may be.

Image links should have an alt tag for the html to be valid and the tag content is going to be used as the link anchor text. So you probably want to use your URL or brand name in the image alternative text.

they have 3 sites and rank the top position for
website for dentist
website for doctor
website for psy
they have only sitewide links from the clients sites they have
all links are in footer, sometime more than 1 with 2 different keywords
When you check those sites all of them have same generic themes and about 4 page some content are not even in the native language, for exemple you are dentist in uk and the price page is in italian

Yes, its still in use. But you have to use them carefully and better avoid them if you are unsure.

Hey OP,
As a few others have already mentioned, you want to ensure that you don't link out using any exact match anchors and instead target just your raw URL as the anchor, that would be your safest bet and will also still pass on plenty of link juice and PR!

Dont do many such links at once, I mean from many sites at the same time, Google does not like such fast backlinks growth. Make sure, that link that you put on some site will stay there "forever", it's very bad when a link appears and then one week later disappears, so post sitewide links only when you shure it will stay there.
