is this over optimization?

It is not over optimization, I do this all the time with my articles and my rankings keep raising steadily. Be careful though to have a good quality article/post and do not stuff it with keywords.

Yes, you should drop the URL and use the server's IP address or something instead.

Look at the top 10 results for your keyword. I'm sure most of them have the keyword in the url, title and some in the content as well. According to me, the keyword density in the content should be kept to as low as possible. But do have the keyword in the url and title. This definitely seems to work right now.

Those are the places where your keywords should always appear, so it's not over-optimisation.

It's not over-optimization as long as your content is high quality. If it's low quality, then no amount of optimizing is going to bring it out of the Google booty-hole...

No, it's not, don't worry about it.

No need to worry. That is the perfect place to add your keyword. But, always keep your keyword density between 2 - 4 percent which is on safer side.

It is not over optimization. The closer the keyword is to the main domain, the better are the ranking chances.
