Are you sure it was really deindexed by Google? Google this search query: site:domain.com. If your website appears in the results, it means it hasn't been deindexed.
Are you using blackhat tactics? Just changing your template will not cause a deindexing of your site in most cases but it might if your new template contains malware, in which case you would have received a warning.
Add your site to Google webmaster tools to check for any manual actions. If there aren't any, submit your site to Google using the 'fetch as google' tool within your WMT dashboard, then build a few links and wait to see if your site will be re-indexed.
Also, confirm that you aren't blocking Google from indexing your site using an online robots.txt checker (google it).

As others have said do the old site:domain.com search on google.
Other than that you could have triggered some sort of temporary sandbox because of the change of template. I doubt it'll last for long if that is the reason.

If you checked the site:domain.tld and you aren't in Google means that you were penalized and you're out from Google.

there could be temporary your ranking drop, just wait few days it will show up.

I don't know how works in Google.com but on Google.it I can change how many times I want all the site and it's always there, in the index. Never happend to be deindexed because I never done spam activity.
OP did you make something "special"?
I have changed my template (WP theme) twice over the last year with major changes to the site structure, and eventually everything gets reindexed. Over the last 6 months, the only links I have been using are tweets and facebook posts. Are any errors showing up in Webmaster Tools after you fetch as G and wait a few days?
But this time my website didn't lost rank,it was removed from index by google

Yes, your site dropped from the Google. Find out and remove spammy low quality links which points your site, then submit reconsideration request to GMT. Take necessary steps accordingly in order to recover from the penalty.

well i said "ranking drop" not "deindexed". i was thinking op didn't know difference between and start talking about deindexed.
