Stay away from blog comment, spammy link exhange especially the ones you pay for.
Contextual links on web 2.0 and others are okay but more effective on original and relevant content.
Don't go over board with too spammy or low quality sites. Sites with manual review are even better or paid inclusion.
If you are aiming for medium competition the best is building your own backlink powerhouse which ofcourse
is pricey and takes time to source but works wonderfully well.

link exchange died couple of years back and blog comments is also a big NO these days and should be never pointed to money sites.

All of the links you mentioned work, it's just a matter of quality.
For example, people commenting here that blog comments don't work. Blog comments can still hold SEO value so long as they're on high authority sites (ideally manually reviewed), low OBLs and you're not doing 1000's of them. Have a read of this for people who quote me saying "BS" etc:

Man your English is horrible I really hope your not the one writing the blog comments.
I simply don't like using blog comments because I have no control over the OBL of the page. If I can create a link there, so can 1000 other people which will make my link go from positive to detrimental.
Blog comments will work when used in moderation provided there aren't 100 Viagra animal porn links on the page. I just don't think there is any way to prevent that from happening.

High PR guest blogging that must be relevant to your niche is what works now a days.
Find authority blogs in your niche, look at if they accept guest bloggers, write good content (with 1/2 links pointing back to your money site) and submit it.
This way you'll get pretty link juice.

Yup, Op you should listen jasons words. The main thing we must analyse the blog before posting in it. Is that a real guest blog or not? Also, Post attractive quality content in the blog. Guest blogging is one of the powerful way to get high quality backlinks. The more links you acquire from the relevant guest blogs to your site will helps you in SERP improvements.
Web 2.0 is still strong if you find some sites with decent PR to post to. You can use all the other methods like social bookmarking and blog commenting as tier 2 and point them to these web 2.0s. You can still do blog commenting as backlinks to your money site but you have to be careful and do that on related sites and do it regularly, a couple of them every day/week. Just stick to it and it should work great.

Yeah, Guest blogging is a real working method of all time. Even Matt cutts has guest posts on his own blog. But be aware in deciding with guest blogs, don't get penalised by spammy low quality guest blogs.

I believe Guest posting is only a way to build quality backlinks. Rest all are counted in spam.

Building PBN, however since so many tries it, its almost impossible to find good domains for it.

For bulk indexing go with GSA SEO indexer/ Backlink Index Express/ Backlink Energizer. If you're going to index one by one, just Google webmaster tools is enough.
