Are there any other obvious sources, other then paypal, to make a purchase from a personal credit card to obtain "cash" without a cash advance credit card fee?

You can use 2checkout.. it is also safe for credit carc.. or you can use Payoneer

I'm in the UK, I've bought US Dollars from a bureau de change with a credit card before.
Gone round the corner to another and changed them back into pounds.
The post office has 0% commission, you still lose out on the exchange rate but it's
cheaper than an advance credit card fee.
