I've created software that automatically creates the proper PHP file for you to do geo-targeting and setting a blank/fake referral. It's coded in Java 
Here's the readme, you can find all the required files in the attachment:

Installing the needed files:

1) Upload globalrefresh.php to your webhost.
Open it with your favourite editor and edit the URL. You only need to edit the domain.
Let's say your domain is w3.blackhats.com, then the line would look like this:

2) Upload check.php to your webhost.
Open it with your editor and edit the second URL (hxtp://w3.whateversite.com/404page/)
3) Download the latest MaxMind IP database: hxtp://w3.maxmind.com/app/geolitecountry
Choose the binary version and save it somewhere.
Extract it, you should have a file called GeoIP.dat now.
Now upload it to the root of your webhost. The file should be called "GeoIP.dat".
4) Download the GeoIP PHP library: hxtp://w3.maxmind.com/download/geoip/api/php/geoip.inc
IMPORTANT: save the file as "geoip.inc.php" and upload it to the root of your webhost.
5) Get CPA Redirector from hxtp://w3.contentgeneration.org/cpa-redirector-version-11-and-framing/
and install it using the provided instructions.
5) Quick overview:
- globalrefresh.php in the root folder, domain edited properly.
- check.php in the root folder, domain edited properly.
- GeoIP.dat in the root folder.
- geoip.inc.php in the root folder.
- CPA Redirector properly installed and working.
Once all of that's done, you can continue to the next step: using the application.
USING THE APPLICATION:

1) Enter the name of the country (2 initials! So US for USA, UK for United Kingdom etc.)
2) Whitehat page: if you want to use a blank referral, you use hxtp://w3.yourdomain.com/globalrefresh.php
if you want to use a fake referral, that cooperates with CPA Redirector, enter your full
URL to the offer (so the special URL with the magic number, like
hxtp://w3.blog.com/new-offers/?mn=4444)
3) Enter blank for a blank referrer, whitehat for the whitehat referrer.
In our example: if you picked the globalrefresh.php page here, you'll want to pick "blank".
In our example: if you picked the special URL with magic number, you'll want to pick "whitehat".
4) Enter the URL of your CPA offer, don't forget http:// !
Obviously, this will only be asked if you chose a blank referrer, since the whitehat referrer
is a link to your blog post with CPA Redirector in place.
5) Keep adding new countries until you've got them all. If you added all of them, enter "N" in the
dialog box that asks you if you want to add a new country.
6) You'll find a file called "output.php" in the root of your C: drive. Upload it to your webhost.
That's the file you'll promote.
OPTIONAL: add an extra "else" at the end of the php file to define what you want to do for other
countries.
