Sorry I can't help, I don't really understand Meta tag, I just put a bunch of relevant keyword on my blog. May be there are others here that understand them well. they may explain it to me too

i would do separate pages for each ie:

Generally, you don't want to repeat any one word more than a couple of times. Also, be sure to work in other terms that might apply (enlargement comes to mind here, and hell even reduction maybe). Also remember that your title tag is most likely going to be what is linked in the search engine results, so it should work to induce the reader to click. Here's what I'd use:

<title>Breast Augmentation in Atlanta Georgia - Breast Implants, Enlargement, Reduction</title>

<META NAME="Keywords" content="breast augmentation,breast implants,atlanta,georgia,enlargement,reduction,surgery,procedure,saline,silicone,safe">

Tack on any other related words or phrases at the end of the keywords tag. Stuff that might be part of a search.

EDIT: I agree about the pipes. A dash or comma works just as well and is more "readable" and click-inducing, IMHO.
