I have no idea how this happened but I noticed when I visited my site my Norton popped up with a threat blocked. I then ran the safe search on it and the following came up:

Threat Report

Total threats found: 1
Drive-By Downloads

Threats found: 1
Here is a complete list: (for more information about a specific threat, click on the Threat Name below) Threat Name: Malicious Site: Malicious Domain Request 2 Location:

1. Most importantly how to I find and remove the code causing this?
2. How did this happen? Was I hacked?

A drive-by download comes from visiting a website who locates an exploit. Could be anything. . no idea. Use a roll back to restore you computer to a system checkpoint date a few days ago.

If you are serious about computer security, forget Norton and use something else.

Try superantispyware as even the free version does better than the bloatware Norton does.

Thanks for the reply. Is it safe to assume that I have been hacked?

Very safe to assume that. Look at the date / time stamps of website files and find the ones changed recently. You'll most likely find the hack in those.
