I do not think it will work...

If someone is upset because of spam...
and clicks on the unsubscribe link...
and a relentless email submit form pops up....
what are the chances that they will be GLAD to fill it in.
and put their EMAIL once again on a spam list..

I have a hard time seeing this work.

I'm sure 99% it will works.
I already sent some not opt-in mass mailing, and tons of people clicks on Unsubscribe link.

And in the unsubscribe page, you slice only the email field from the offer.
Just be careful with your CTR... Nobody clicks on "Unsubscribe" by curiosity.

you just have to write..
"You have ask for unsubscribe from ...., for complete just enter your email below
EMAIL SUBMIT
Thank you"
