IMHO, By using your categories in your titles, you give extra power to  your web site. Think that you have a site dedicated to health issues. A  post named "Making exercises" falls in category backpain.
If the exact keyword is placed in the url, I don't see an explanation for using a category with it in PL structure.

Though IMHO trying to rank only one kw is better than targeting two(Per post:- For new sites)

i beg to differ

you cannot "justify" a post to come under certain category by every blogger , then how will SE understand the "relevancy"

You may be right if we are talking about long-tail keywords or niche related websites. But mostly, websites will be bigger having lots of categories. Think about a movie site. And I have categories as movie names. Then what is the point here if I do not use the name of the movies in my title? Then I would give up declaring those names to search engines and lose lots of traffic.

By using those names of movies in my categories, I also guarantee that I play for the searches related to them.

Anyway. I am always using them this way and very happy with it. I also used only the postname option and after two-three weeks, then moved to the structure with category name.

In short, I may say, by not utilizing category names, you are missing lots of search queries.

Maybe its Good for big content farms, broad searches and blogs with high content but not for small blogs and when you are fighting for an exact match term

just one question lets say you do all these steps, and in time, your blog gets loads of traffic.

will the hosting options you mentioned be able to handle high traffic coming to the site? I often hear of people starting with the cheap hosting, etc, then when the high traffic comes their forced to upgrade.
 
I prefer to make a regular page with privacy policy to keep the no. of plugins down to a minimum.

To add a plugin to your list can I recommend Sharedaddy and StatPress Visitors.
 
I was wondering about plugins as well. Is there such a thing as too many plugins, might it create some conflict between them?
i also use %postname%, only because a lot of the more knowledgeable people on here say it is better for niche sites. Very nice guide by the way.:D

Yes this happens sometimes. But my list is OK. I use 30 more but I will not name them here since I would like to keep them to me.
