I Need This FB Script

I found a CPA Landing Page script from CPA LEAD that I want to use on my wordpress site. The link to the example is below they use a simple box asking for an opinion and inside that box is your CPA ad, and your Facebook Fanpage "Like" Widget. This script allows you to make up your own questions and surveys and once the visitor answers your survey they get linked right to the CPA AD. Well I understand its a landing page script that incorporates a survey and a facebook "Like box" widget inside of it. I just can't find this script anywhere and I can't find anybody that can do this for me and it looks simple. The script allows you to make up your own survey questions too. Any ideas on where to get this>

that "facebook widget" is a standard widget that can be found on any fanpage management section
