Lately I'm seeing a very consistent pattern: new ad accounts get approved and run normally for the first 3–6 days, but then suddenly get disabled around day 7 — even with slow warm-up, clean creatives, simple campaign structure and proper billing.
It feels like Meta has changed something big this year.
For those still running ads successfully:

Lately, there have been a lot of account suspensions due to Meta’s new algorithms. Only high-quality advertising accounts can save the day, but in our experience, even accounts with five years of history have been permanently suspended. Ad accounts have always been blocked, so you need to treat them as disposable and have backup accounts, as well as Business Manager accounts where you don’t run ads but only create pixels, so that the information from the pixels is stored separately and doesn’t get lost when the Business Manager is blocked

have you checked your device IP yet ?
if your IP has been flagged by meta then no matter how many accounts you use they will all get swept

we only run ads for clean products yet ad restrictions still occur is this because the accounts arenot strong enough in many cases even with wh products a low trust score or a weak account history can trigger the system's automated flags especially during strict scanning periods
 
I ever seen similar reports, but there's rarely a single trigger. A lot of accounts pass the initial checks and then get reviewed again once enough activity, spend, and behavioral data has accumulated.

Yeah, I’ve seen people mention this too. I’d pay more attention to the account’s trust signals than just the warm up, business details, billing history, domain and Account Quality can all matter when Meta reassesses an account.
