Forms been doing great lately I just keep them super short as name, phone, that's it. Adding too many questions kills CVR fast. I also tried different titles, they seem to have a little effect on conversions

I removed the greeting and next thing I know my CPL has decreased significantly! Just need to make as free from friction as possible (marketing 101 LOL)

That's probably true. I also had the same problem with the form being too long and they left halfway through. Shortening it and changing the questions to lighter ones made it much better
 
I think it still works. Refresh the content, shorten the form, make the offer more attractive.

You need stronger incetives in order to boost your forms's CVR. You have to give extraordinary value to people in order to fill up your forms (it depends on your niche as well).
