Need an SEO performance report template

Hey Guys,
I have never made reports on SEO performance for clients, and I have no Idea what to mention and what not to. Can someone please provide a template? Or an Old performance report of your own.

I tried getting it online, but almost all are too complicated and have so much unnecessary information. I need a simple report of monthly SEO.

Excel or Google Sheets would be appreciated. But anything would work.
