Bot or Script to Fill Forms Using My Data and From Proxies

I need a bot or a script that will allow me to upload a csv file of a contact data list.

From there I want the bot/script to fill out the form on my site using the data from the csv file and a proxy list.

Can someone point me in the right direction to find something like this? I think I can use Magic Spinner but I havent used those types of app in a while.

Any help would be appreciated!

You could use your own script because from the sounds of it you might require some modification in the near future too.
