Has anyone botted the app store and have any experience with getting flagged. Was looking to do downloads/reviews, just want to make sure that they won't flag the app. What's the worse case scenario for botting?

A lot of spam reviews/installs will lead eventually to the app getting delisted and the account may get terminated.
Companies that provide those services (install & reviews) uses GPTI apps & services to delivers them, & some use phone farms, google monitor everything from google play ID, the devices IDs, IMEI ...etc.

That's what i figured, do see why Apple wouldn't quickly filter that out.

in some cases they ban the keywords and suspend the account later

Currently we are using Whatsapp Business APIs to send service based messages to our registered users who are using our service, we need to whitelabel this account so as to never face any spam/block/flag issue which in any way may effect our services
( like we dont want them to block our whats app like some users may report it as span despite putting all the disclaimers and following every rule. is there any measure to tackle this )

Whatsapp has a Business verification program (green checkmark next to numbers) , but it's on beta I think, and It's limited to few countries.
If you only send Transactional messages to your clients, I believe you'd be good. If you are doing cold outreach campaigns, then better have a software & churn & burn numbers, & never links them back to your original business/domaiin.
