So judging from your screenshot it looks like you got nearly a 1% clickthrough rate to your clickbank link. That actually seems to be pretty much the standard CTR for contact form spamming. Regarding the failed rate, check your captcha solver and proxies, or look into cloaking your clickbank links.

Well that would be down to email copy and how targeted it is for the contact forms he's submitting to, along with the quality of the clickbank product being promoted. Remember, cold email and contact form spam are some of the worst converting traffics on the planet. Have you ever bought something that someone spammed through a contact form?

It's better to build an email list first by offering them some ebooks, PLR products, etc..
And after gaining some trust, then throw some product offers to them.

Yeah to be honest I've tried lots of affiliate programs like clickbank and the likes and never had much luck by cold emailing or form posting to them. My opinion is that those programs ae generally done to death so they meet with reader apathy pretty quickly. My advice to anyone would be to create your own affiliate program which is what I do. I will contact website owners in certain categories where I know I can effectively get sales and ask them if they would be willing to pay me a commission per sale. This works well for me.

Contact form spamming is one of the most annoying things any webmaster has to deal with. I cannot imagine anyone makes any money from this type of spam.
