No, monthly. And I believe it's sessions, not page views. You will have a hard time getting in most ad networks if you don't have 80%+ from the US, as this is the traffic business are willing to pay for.

If you don't have a lot of page views, you can try Monumetric (I think they require 10k page views per month), but they have a one time setup fee of $99. Always enable video ads to get higher RPM.

Ezoic seems to be different from case to case. Some say it slows their website down a lot, while others report low EPMV/RPM. However, they are the most common when it comes to smaller sites.

There's also another one called NitroPay but I really don't know much about them.

In addition to some of the ones that have been discussed, you might also consider giving AdMaven a try. It's a well-regarded advertising company overall, and its inventory spans all kinds of verticals and themes.
