How to create custom form in a page in wordpress?

So I need to create a form on a page in wordpress. Something like this:



Creating a form isn't an issue. I could probably use the Jetpack contact form (which I already use) to create it.

The problem is the dropdown and the ability to display content in Step 3 based on what choice was selected on Step 2.

Any ideas?

Actually, I have this HTML site where I have something similar.

So there's 5 tabs on the page, and depending on which one you click, different content is displayed.

But the difference is that I don't know if it can be done that way and have the email I receive contain only the content from the particular tab that was active at the time the submit button was clicked.

So I guess I'll have to use a plugin then. Any suggestions?

I checked out Ninja Forms, but the plugin is stuck on "Checking connection... " and I can't do anything.

Delete plugin...

Gravity forms and I believe Contact Form 7 allows this as well if I'm not mistaken.

GF is perfect for this kind of thing though.

I tried using the Jetpack contact form. And I can make it work actually, but it bothers me a bit that I can't get some basic functions I'd like (conditional fields, simple number validation).

From the googling I did to fix up the Jetpack form, I came to the conclusion that there's a lot more help out there for Contact Form 7. So I'll give that a shot too...

You may be a bit late to the partay, and I'm sorry but no free entrance for late comers, unless they're hot chicks, so you have to pay for your ticket too

But you may be the winner here. No one else recommended Contact Form 7 here. But I'm playing with it now and it can do everything Jetpack contact form can do, and more. Number validation, done.

So I had an issue where the new fields I added on the contact form, the inputs and selections were not arriving in the email when I get it.

Had to switch to the Mail section in CF7 and add the fields to the Message body.

Still figuring it all out, but I think this is gonna work out.
