1. It’s hard tbh, and directly related to your second question. First of all, try to find out the reason Google penalized you and then only members here can suggest you accordingly. But overall, in most cases, as soon as Google penalizes, it’s better to leave the site and start over with another, unless that site is too valuable for your purpose of course.
2. Multiple aspects are involved in the vague “spam score” term. But assuming you are talking about the obvious backlinks, then the only thing you need to do is remove all the spammy links you are aware of and start building only quality editorial links gradually. Even if you can’t completely get rid of previous bad links, you can still build quality links hoping someday they will outperform the bad ones. The process is tedious and time-consuming, you've been warned.

Some penalties can't be lifted. Others can. It requires link cleanup and content improvement. Also takes a long time, if ever.
Sometimes it's just easier to restart with a fresh domain.

I lifted some before but Google does prefer people with ferrets I suspect they have secret policy in place called ferret policyYou sure?

This applies to most of the cases excluding the ones where a website was getting 5 figure income per month, I think

1. It depends on the kind of penalty
2. Remove high spam score links + build more links with low spam score domains.
Some may tell you to disavow those links but it makes no sense because
MOZ doesn't know what you submit on Google Search Console.

You shouldn't really bother with penalized domains unless you got one with really good links pointing to it,
double redirections do work in some cases while not in others, it's just a gamble,
for the spam score, you should actually ignore it, Moz is way behind being a decent tool to rely on their metrics in any kind of ways
