How to fill up google ads business operations verification form?

hello BHW sensei,
Last night i got a mail from google ads about business operations verification and they paused my all campaign. I dont know how to fill it properly.
i don't run any business or company. i just promote affiliate products (health) via google ads. i use only website and landing pages to promote products. so please guide me how to clear this up.

My account type is individual. Thats why I am confused

that's strange. I suggest you contact google ads support or email them they will definitely help you.

I already sent them a mail regarding this issue. Waiting for their reply.
I searched everywhere about this issue but didn't get anything.

Churn & burn saves time and money nowadays. That is my experience and my opinion. People may have various different experiences and a variety of different opinions. Even legit GAds accounts running WH with good spending history are getting permanently mass suspended nowadays & many such affected people have mentioned the same here at BHW.
Though yes, I understand that your question is what you must fill & submit pertaining to that form, so yes, I do admit that I do stand guilty of not exactly aligning my reply to your exact question, however I have given you a possible alternative way out. Different things work differently for different people, based on how various people implement various things and hence results may differ.

In my experience there are aged accounts with spend history that work for 6-12 months before getting banned on BH niches.
With legit spend.

You can still get banned even if the account is 10 years old.

I guess it all depends on what your niche is, and how good your bh ad copy or keywords are.

I believe the same. I got a 2008 account suspended once which had spent $250k in it within 10 minutes of ad campaign creation, and on the other hand, some new accounts worked for me for months, before getting banned. It all depends on your ad creation method.
