Yup, use a very lightweight theme and don't install unnecessary plugins.
For example, I use the GeneratePress or Astra Pro themes for almost all my websites, they are lightweight and easy to extend.
Nah, I don't like it. I do go with the OxyGen theme builder or Elementor Pro if you're looking for a more out-of-the-box solution.

Use an optimized host such as ServeBolt or Cloudways (with Vultr servers), make use of MariaDB, Litespeed, servers.

If you have an old backup, start from there and scan it with WordFence, or Malcare.
Place it on a staging domain and see if there are no malicious files created anymore. Change all passwords (and all the other security measures) and consider if you would like to start again or use this version.
I personally prefer to start over again and export/import all the data using WP All Export / WP All Import. Mostly, theme or plugin files are infected, not the content of posts itself (from my own experience).
