AdSense [India] Wire Transfer - Need Information!

Anyone from India using AdSense and calling payment through wire transfer?
I need some information as my bank is not helping me out!
FYI, this is my first payment.

Visit your bank,you have to sign and give a form to your branch to accept international payments.

Also there is an additional form, fill that too for future payments.

Both available at branch?
I visited them twice already, they said send mail to customer care or call them. I did the same, but they sent me a link which do not provide the information. Customer care guys told me to visit branch.

Will visit again and settle!

Yes, both available in branch.

You need branch ifs code, swift code & your account number. Go to your brach, definitely there you will see a board that shows its ifs and swift code. Every branch has a publicaly visible board with such details. What else you need??
