KGR is allintitle results divided by monthly search volume for keywords < 250 searches a month.

It still works for some keywords if you take the KGR and put it in Title + H1 + good content.

Yes indeed it still works.
I am currently using the technique on a couple of old WZone sites and have now gained some long-tail rankings after doing nothing more than optimising URLs, page titles and headings to include researched golden ratio keywords.
