I particularly like Braxton, but it could be any theme of your choice.
Plugin using own layout for quiz pages, so whatever theme you use it could be adjusted what layout you would like to use through Settings of plugin.

No immediate plans, but it's on the list of addons we'll be working on in the next few months, however, most probably it would be available only to multi-site license users or as a paid addon.

In your wp admin go to plugins - add new and click on upload.

If you're having problems when you upload the zip file to upgrade / update due to "already exists" error message then install this plugin: https://wordpress.org/plugins/update-theme-and-plugins-from-zip-file/

Simply activate it (there are no settings to adjust) and you will be able to upload the quize plugin .zip file and it will install.

I was never using it before so I don't know if Featured Image was never there or has somehow disappeared (I also checked in "screen options"). Reason I would like to have a featured image so I can list quize posts with an associated image and make them look more interesting.

By default quizzes not published on the home page. To have quiz as a post you need to create a post assign quiz to it and add any featured image of your choice.
