Phone verification is not bad. It is a wonderful system of keeping your social accounts safe and secured.
Oh, wait ... Do you mean : Usage of 3rd party phone verification ? If yes, you need to rephrase your query.

If you keep getting hit with having to phone verify then yea that's not a good thing. Rather than a random check, it's more likely you're triggering something on their system to ask for you to phone verify your account.

I typically don't have trouble but within the last week about 50% of my accounts have gotten hit with PV. Some have gotten PV twice, some accounts not at all. But I thought it was strange that all PV happened in a short period of time (about 1 week).

Ontop of that, last night....8 accounts of mine all got PV.

What could this be, is it cause for concern? Is there anything I should do?

Yea that would definitely be cause for concern. Most people will tell you the proxies are bad. I'd say be careful what bot you're using and what actions you're performing. If you aren't changing the hash of each post before uploading then that's a common reason for getting hit as well.

So change proxies? Also I don't use hashtags with pics on the accounts that got hit with PV.

yes, when it asks for verification some pages I can verify, others I need to put a 5 numbers until arrive the sms, and others take ban as soon as I confirm the code.
I'm running 50 pages at the moment, I lost 15 because of this problem

What do you mean " I lost 15 because of this problem"....did instagram delete the account or are you unable to verify?

yea ive been having this same problem as well, got hit with 5 pv in 2 days. just decided to shut down the account.
my bot setting were REALLY low, like 19 follows in 1 day in between 8 hours and thats all. Still got hit one, and i dont think its my proxy
provider cause i use the same proxies on 2 different accounts and they have to be hit.

I had this problem a few weeks ago as I am running quite a few accounts too. What I did was give them a rest for a week after having confirmed them, then I warmed them up again. To warm them up I was EXTREMELY careful and gentle. I first had the accounts only view stories, around 20 a day, and not like or follow anything. then after another week I had them start following around 20 accounts a day, then I increased it by 5 followers after every 3 days. After a week of that I started reposting content again and started making it like only pictures from current followers and nobody else, no more than 50 likes a day. Everything is running smoothly currently and I haven't had a single PV or Email verification, or even action block since. Just be patient and take your time, and make sure you make the account look like it is being run by a real person. Ramp everything up gradually and things will be fine.

Great, what did you do to get past phone verifications? I know you only want 1 number for every 2 accounts and it is not wise to "juggle" phone numbers across accounts.

I looked in BHW's marketplace and found many good SIM card sellers, then I got an unlocked smart watch from Aliexpress that can use SMS. That is how I verify my accs. I will not recommend any specific seller, but just make sure you do your research on the different sellers here in the marketplace and find which one works best for you.
