In the description you should simply explain about what you're giving them.

In the website you should put in your website that leads to your landing page or your affiliate offer link.

You're most likely allowed to get paid out with PayPal but it all depends on your network, just go on "Payments" tab or something similar to see if they offer payouts from PayPal.
