Only way to be safe is to format your drive and install a clean OS version
You're probably RATED. Do this:
1) Download MalwareBytes
2) Install and then DISCONNECT ur internet connection
3) Run MalwareBytes and scan ur PC
4) If you don't have Anti-Virus, download it, i recommend you to use Kaspersky
5) DONE
You should be safe now, make sure you never download any pirate games or cracks/keygens, etc... Most of them have RATs in it.

Clean OS may only take 15-20 minutes but what about backing up important files and documents, what about reinstalling all your programs, finding/retrieving license keys!
It all takes time and it may be a simple problem to solve so exhaust all other options first!!!
If you don't have a separate partition for system only, then you are in a great risk.
Important files and documents should already be in another partition. Reinstalling programs is the main idea of the whole process, getting new and clean files would take another 15 - 20 minutes. No idea about license keys, I install pre-active OEM versions
Anyway, executables is the main disadvantage of windows. If you think every virus is there to announce about itself, u are very wrong. Most of them are hidden and work quietly to log ur data, gather info, or just wait for new orders.
I have been in penetration testing and exploit development for so long. It's very rare to find a really clean pc, and after all these years, formatting the C:\ every now and then is a very reliable solution.
I have a separate partition for documents but sometimes in a hurry you don't move everything across.
It's reinstalling all the programs that can be the issue, I personally have dozens on my pc that I use, many of them require license keys so I then have to dig out my backup of those. Some of them (hint:adobe) take an eternity to install.
Maybe, I needed to elaborate more on now and then part. Installing new windows once a year is a good estimate. So, even it takes 3 hours to get everything up, it's still nothing if you do it once a year.

I've cleaned dozens of pc's for friends and family and they never have the issue return.
There are hundreds of different programs designed to eradicate spyware, malware, browser hijack, rootkits etc
Clean a pc thoroughly and you'll be fine.
