I know some guys that run ads on the same account, for 7 years in a row. They get instant approved etc. They spend 500k+ on ads. It's a combination of age and spent etc.

Yeah, that is what I was thinking. Even the page name is not legit. The guy is doing it the blackhat way...

Facebook would never approve this for even the largest accounts.
Whatever domain is being linked to, it will have be cloaked. I suspect how it's done is you submit and run it as a whitehat campaign for a few days. You then edit the campaign, changing the image to what's seen above. Often Facebook will auto-approve the campaign again as long as it's not flagged up by their detection systems.
When I run adverts, I play around with the images quite a lot and only sometimes have to wait for manual re-approval.
Seen it done too, run a 'normal' ad then switch it over to one that wouldn't have got passed by a review.

The images resembles a typical picture used in biology class lesson so Facebook passes this.
The wording - > I suspect the text was edited after the ad was approved.
Cloaking was used to hide the landing page.

facebook ads are serving to user's interest.... if that ad is about peenis enlargement.... means it is your interest and you been searching about it???

Yes. He might have ran a campaign, paused, changed the image. I don't think his accounts will last long though.

If you have been running ads for Years, actually, almost all your ads will be approved.
