When a proxy dies what happens to its ip? Does it remain dead forever or does it eventually become assigned to something else? Like a new website, server or does it always get recycled and used for a new proxy?

The internet is not running out of IP addresses. They ran out of IPv4 addresses and thus made IPv6.

The IPs never die, proxies do.
An IP address range containing a huge lot of individual IPs (normally more than a thousand) are lent out to internet service providers or large enterprises that have backbone access to the internet.
The organisms regulating bulk IP addresses attributions are Organisms such as ARIN, ICANN and others.
When you use a proxy, it's either public, shared or private.
Shared and Private proxies providers are basically little "ISPs" and lend you one or more IP address for you to use freely on a specific port. The proxy "dies" when you decide to cancel your subscription, but gets recycled to another customer eventually.
Public proxies aren't supposed to be proxies in the first place, they are simply IP addresses with unprotected ports in which we can route our traffic into.
Depending if the IP address owner has access to more than 1 ip address, he may change his IP for another one in his pool. His old IP will then eventually get recycled by his ISP or Network and get attributed to someone else, which may not have the same open ports, thus "killing" the proxy. If the ISP decides to give back their lent IP addresses ranges to an accredited organism, the IP becomes unused for some time, until it gets lent out again by this very same regulatory organism.

not sure why some ppl here are saying they are not...
a proxy's ip dies, it may be picked up by another proxy, may be given to a website, or could be given to a mobile/desktop...
They usually stay dead for a while, but someone will pick it up.
The reason we are running out of ip4 is that there are too many users at once. (I myself own around 15 ip's in constant use) there are only 4,294,967,296 ip's in the world, and a hell of a lot of websites/proxies/users

A proxy server is just a service running on one or more ports bound to a given IP address. Some reasons why you can use the proxy server is because:
A) Misconfiguration - at some point someone will spot this and fix it, it's not dead - just not accessible without credentials/from a certain IP
B) Blackhat - someone has gained access to a server and setup an open proxy - again will be fixed at some point, taken offline.
Try pinging or port scanning (be sure of the laws in your area before doing so) an IP that you used to use as an open proxy, and no longer can't. You'll see it's there, just doesn't have a proxy server running on that IP address anymore.

Interesting, so the can be re-cycled.
I always wondered, i receive tons of emails from my sites with the clients IP adress in the message header. Could i use these ip's as a "private proxies"? I don't know how to find the working port but could you use them if you know?

IPv4 ip space will never die (or maybe will dissapear in about 30-40 years, an IPv4 ip that has been used as a private proxy can be used after as the dedicated ip for a SSL certificate, or a dedicated for domain, hosting, VPS, dedicated machines, DNS, etc ...
ARIN is not giving anymore ipv4 ips ...
a new world is coming and is called IPV6, and in about 2-3 years we will be out of IPv4 ips, and we will be forced to use IPv6, also major websites like facebook, google etc .. have already implemented IPv6, and you can access this websites using IPV6 IPs ...

NO you can not use them as private proxies, they are called "private" because they are restricted to work on particular IPs, or they are protected with user and password ...
