In terms of ranking and seo, when a person says add fresh content periodically what exactly do they mean? For example I deal in mini sites, 4-5 pages. Generally EMD

So if my domain is Ailment Relief.com and my main page displays an article related to "ailment relief" with the keyword used 2-3%, and the other subpages have similar and relate-able topics, and my main goal is to rank high for Ailment Relief, am I adding fresh content to the Ailment Relief article, or am I adding another article all together i.e.
    am i adding content to www,ailment relief.com
    or
adding an additional page/article all together at www, ailment relief.com/newarticle

hope this makes sense, thanks in advance

"fresh content" = "original content" (or, for some people who spin - as original as possible). Something you write yourself would be totally original, for example. I would do both if I were you (more work but it will pay off).

Thanks given, but I think I mis-explained the question. I know what fresh/original content means. My question is where are they referring to PLACING it, on the index or just adding it to the site in general (a new page after the slash)

    Home Page:
I've wondered this too. I think you get credit for other pages that have new content, although it certainly helps if your front page changes too, like with blog content.

I think "fresh" is also supposed to mean unique content (ideally)...not just an RSS feed from other sites, although that's one way to fake it if you can't get your own content.

I agree, fresh content means "spin"-free, original content that's so good that, maybe, god willing, someone will actually, naturally link to it.

Or it could mean the hip-hop kind of fresh, which is is similar to dope, or bad (bad meaning good).

I think this is the area I suck at most. I tend to make a site with 15-20 articles and hope it ranks with some link building. Still have never hit a #1,2, or 3 position. Time to focus on adding tons of new content to my sites.
