Are you using broad match keywords?
I would think you would need exact match and then an ad specifically for that keyword to reach that level of CTR, no?

Maybe or using broad match and loading up the negative keyword list.

I use broad match and spend more time on researching -ve KWs as @coolsheet said, since these articles are the informative so. and I use ad extensions so that increase my CTR

For 42% CTR most likely the ad campaign is targeting branded keywords

Some websites have 1.5% - 2%, while others have 7%-8%. Getting +10% is an incredible result.
