Depends on the age of the website.

For a new site, 5-20 links per week with a lot of link juice

If the site is aged, you can blast it to hell.

A couple of PR home page links, backed up by a lot of web2 links, throw in some wiki links followed by some top quality article links, such as ezine and then the best bookmarking links. Keep all the content on each unique and repeat, plus throwing some good quality links in, then if you like smash some Sape links at it, as offered by recommended people on here like Tommy. Throw 40% of your Sape links at your web2's, articles and bookmarks, the rest to your money site. Repeat the process every month and you should rank.

Oh yes, whack a few social signals in for good measure. If you cannot rank a site this way, move on t a profitable niche which will rank this way. This is near enough my method, and to say the least, I live off it.

Mix all your links up, use good bookmarks and high pr blog comments on everything. Never try to follow a pattern,
just mix it all up, with the best pointing at your site.

IMO the best for 1st tier would be: press releases, files/slides sharing, high PR homepage links, yahoo answers, niche targeted contextual links from guest posts/forums/blog comments, some high value edu/gov etc. For 2nd: private blog network submissions, web 2.0, wikis etc

For 1st Tier I am always going for web 2.0 simply because it's the simpliest way to get contextual backlinks which can be changed/remove at a later point. Social Bookmarks and Social signs are needed, are fine, but the solid link juice comes from the web 2.0 properties.
