What is the best way to collect phone numbers from a Restaurant

Hey guys,
I landed a new client sometimes back and I need to send a proposal of what I'll be offering etc.
I had a meeting with her, and we agreed that I'll give her a list of all the services that I'll be offering to market her restaurant.
I will offer her ORM, and text message marketing.
Now I have some slight problem, am undecided, what is the best way to collect phone numbers of the actual people who eat in that restaurant so as to send out the coupon deals?
 
Advertise text message coupons at the restaurant. I have seen places use an iPad and a stand to have instant sign up there at the store. For cheaper options, you could advertise via printouts and leave them on the tables.

I design a review card where the customer fill their name email, phone number then they write their reviews.
Which I can post later online

You could do that if you're posting the reviews on your website. It may be a good idea to have a card that shows customers where to leave a review online (Yelp, Google+, FB, other local directories)

You answered your own question. I think this would be the best way to go. They do that at a lot of the resturaunts here. Just stick a nice looking card in the bill, and you should be good.
 
Why not setup a seperate page for the resturant where people can go to signup to recieve the coupons , Then get a budget of say $500 and get phone numbers and openvbx system or something similar and blast out a message to people to go and signup at your site. You will get a bout 500 people in no time this way.
 
An actual survey conducted in restaurants can help you.Marketers always do that,in the form of satisfaction progress report,etc.
 
Just ask do they like to have "discounts" , most people like discount. Ask them to fill the form with phone number.
