User submitted ratings and reviews on Wordpress. Need theme and/or plugin

Hey all.

I want to make a Wordpress site with user submitted reviews.

The site should have the option for a surfer to give a 1-5 star rating as well as write a small comment. Which themes are prepared for this, or which plugins do I need?

It would be beneficial if:
1. Reviews would be put in moderation queue before going live.
2. The theme is from a proper provider (Kriesi, Elegant themes etc)
3. One could blacklist certain words in a review text (such as http)
4. One has the option of mail verification before a review goes live.

Any ideas?
