Can someone help me with formatting bulk important account upload into twitterdub?

I want to bulk upload the username, password, email, phone number and authentication cookie token thing.

Ive spent hours trying to format it and when I upload it, it just doesnt import correctly.

Had the same, click custom import, not import and that's all
And be sure you have correct lines like

username,password
deeznutzulick,123456
...
...

I had same issue, btw solved thanks to Martin's reply (rootjazz support email).
Template in CSV attached here as a screenshot. "auth" is not mandatory, but good if you have it.
Use "Custom import"
