I need one script to sell a kind of service.

So visitors fill in a form, pay (paypal and bank transfer) and have access into a membership page with support option.

An affiliates option will be welcome too.

buy vbulletin, you can create memberships, offer support. Dunno for the affiliate thing but surely you can add a third party code somewhere.
