Basically I'm in the process of redesigning my site, or at least I want to.

The way we are going to have it set-up it's going to change some of the urls.

    http://hotels.com/las-vegas

are being changed to this format

    http://hotels.com/nevada/las-vegas

Now, all my links where build to the the original link, if I change all my links to the next format, will this affect my rankings?

Yes, the moment you change your URL structure, all your rankings would be affected.
But remember to redirect all those old links to new links or else your readers might get 404 error while visiting the old link

I'll just add on top of this -

If you're using Wordpress. there is a nice plugin for that called - 404 redirected
Also you can make a redirects from .htaccess, which is sometime even easier.

This way you're not going to lose the juice ;)

What the above suggested will redirect from the 404 page to the homepage, you might not lose much PR there but the new urls will still need some work.
