I am ranking as number 1 in yahoo and bing for a local medical search term but just barley in the top 50 of google. Does google penalize if I don't display that i have a degree in the appropriate field? Or what else could be going on?

I think you're looking too far into it, and giving Google too much undue credit.

Google and Yahoo have pretty drastically different algorithms. I myself can't speak to exactly what would cause you to be ranking in those search engines and not G, but often times I've found that when I'm ranking well in G, I don't rank well in Y and B - and when I'm ranking well in Y and B, I'm not ranking well in G.

What's your BL profile like? Any spammy tactics in use? Have you implemented Google authorship/publisher markups?

Wow, you're really looking in to it deep. That's actually a cool thought though. Google "not" ranking a medical site due to no proof of a degree. Genius.

My back link profile looks 100 times better than my competition imo. My competition are actual doctors in my keyword groups and have kinda spamy looking BL profiles. I came across this hypothesis from looking at basic SEO Tips for medical sites. Everything I found basically said to list doctors and staff with pictures of them. I don't have this on my site, but eventually will once I get clients. I just really want to rank this site before getting clients so I can "prove" that I know what I'm doing. My be I'm just getting to impatient and anxious, but who knows. Google has never really been a good friend to me.

If it's local, go for local and high quality directory listings. Many of these will be paid.

There's some good info on them in a few threads here.
Here's one that will outline a good approach to local SEO.
Read through the posts by Techxan. He's knowledgeable on the subject. As is Nigel Farage.
