Latest update: The problem seems to be with my htaccess file. When I delete it from root, everything functions normally. when I add it, the problem below appears.
my htaccess has 3 things in it: index.html to root, non-www to www, and deny a bunch of ip addresses. could it be i am denying too many ip addresses and it's not liking it?

I have got a problem, I am wondering if anyone can guess what's going on.

I have 2 add-on domains under my host. I installed fast secure contact form for both of them. for site #1, it works fine.
for site #2, it worked fine about a week ago. then, today I decided to change a field in my contact form and went to site2's contact files admin page. it would not let me log in. my password would not work. Then, I tried the forgot my password thing. reset password, password still would not let me in.
I am very familiar with this script and I know when the password is wrong, it says in a red background "invalid password" but for this site, it doesn't say anything. when you press log in, the page refreshes and you are presented with the same log in screen. no errors.

then I deleted all files for that site and started from scratch and installed the contact form again. the form has a "default" contact form with 4 fields, which you can change, you can delete any field, add any field, change background color, etc. while I was in the admin panel, I made several changes to the form, background color, different fields, and pressed "update options".
normally when you update a form and press "save options", the page refreshes and you get a message "options saved" but now it does not show me that message. The page refreshes and all the options are back to default. The background color is back to default, everything is back to default, it will not save your changes.
then I logged out of admin and tried to log back in, and I cant log in again.
this is a pure php script, no mysql involved. I really dont know what the hell is going on, the problem is not my host, I have another add on domain with them and the script is behaving normally, I just spent my entire day on this problem...I dont know WTF is going on. Any guesses?

Can you get access to the Apache error_log file? The details of your issue may be in there.

there is an error_log file in root, and i just downloaded it to take a look. it's just an error from a file not being found...this is from before and is not related to this problem.

the problem is 100% with my htaccess, every time i delete it, I can log in and save. if I put it back in, I cant log in.
WTF, why would htaccess conflict with the php contact script?
