There's an offer on Peerfly that converts on a $2.95 payment which apparently makes you almost $35 per lead.

I was wondering how this would this be profitable for the advertiser. Is there a huge up sell or something?

a rebill that bills at 69.99 or something after 3 months. Most people forget to cancel their trails or what not and bam advertiser just banked about 30 bucks.
