    I have a bunch of pages that I link to internally to but don't want them indexed in Google.

    Had the same situation with another site and did this:

    - robots exclude those pages
    - noindex on the pages
    - nofollow in links that point to these pages

    Yet Google STILL indexed these. What's the proper way to get pages not indexed?

    Use php code to detect a bot/spider and give them nothing at all - preferably a 404 error. That is the only way to be 100% certain. Not all robots/spiders adhere to what you put in your robots.txt file.

    @OP: Submit a manual removal request via webmaster tools (in addition to your existing steps, as Google requires robots.txt exclusion as well).
