Need Mobile Landing/Squeeze Page

I need someone who can make a high quality landing/squeeze page out of woomco.com/ppc-1

This is the template we are using.

If you have a squeeze page already wehich we can add our text to it, and it has a good mobile version this is an option as well.

This is for a real estate so the theme either needs to be plain or real estate based.

Also looking for high quality lead captures including pop up lead capture. It is hard to capture leads through mobiel sites so I am thinking of using actual lead capture on page AND using pop up lead capture.

If you can either create a mobile page out of the page I have provided with some customizations for lead capture or have a nice theme with mobile lead capture/pop up lead capture please contact asap
