Need someone to recreate me this page a form that is so simple:
Input data
Hit create
Allows User to print print it, save it as a pdf and email it or whatever
FORM IS SIMPLE! Source code is able to be inspected. I am not looking to get robbed and I know I can get it done cheap on DP or GAF but...I do it for BHW.
It needs to be copied box for box, form for form, option for option. Not to hard
