Use captcha and if your site is in wordpress then you can download some specialized plugin to catch spam comments.

Just disable the comments. I don't really even see a purpose for them anyway. It's basically only for spammers who want backlinks. Either do that or make the comment section no-follow

I have the comments turned off.

I don't like them at all. If people really like your post, then they share it and thats you really want.

Turn off commenting or akismet plugin is one of the best option for it.

Fake comments are for nothing. Interaction on posts is not a ranking factor in Google, trust me.
As many people advices here, the best thing you can do is to turn of the comments. Take that action and do not waste your time moderating comments.

Use Akisment and (I believe) Disqus has a good anti-spam system built right in, it's also a fantastic comment platform that integrates very well with WP.

No way I would leave a comment on a post using keycaptcha.
That's to much "effort to ask for" from your visitors.
Feel free to prove me wrong, but I'm sure the amount of comments would drop.

Stopping spam comments (well 99.99% anyway) is easy. All you need is just a little javascript on the page that modifies the form in some way that must be present on the form submit. All comment scripts (next to all) will not process javascript, so the form will fail.

No idea if something like that exists for WP (assume you are using), but take a look

Fake comments are for nothing? Think about it! Most people who won't trust in what you are offering will be convinced and gain trust after they read positive comments like "Oh great, it worked!" and stuff like that.
