Reach out to online communities that you believe would be interested in the design. Sign up for forums (Tapatalk is a great free app that can help you find these) and participate in social media conversations/pages.

A landing page or ad with attractive female Doctors would be good but before doing anything you need to get somebody to work on the copy on that page:

I'd also like to note that your message is way off target. Who will buy a shirt like this? A real doctor wouldn't be caught dead wearing something like this and any College fraternity John that wears this wouldn't be mistaken for a doctor.

The shirt is essentially encouraging women to sleep with doctors. Is that an area of life that most doctors need help with?

Perhaps you could find a way to make it funny by suggesting that the wearer of the shirt is a doctor.

The back (or front) of the shirt could say something like "Trust me, I'm a doctor"

You're doing it wrong. With teespring, you don't design a good shirt first then look for potential buyers later. You have to identify your market first, research for the most passionate audience (think of Pinterest, Twitter, FB pages, groups, forums, etc), then create a funny or controversial design for them (memes). You can go with free traffic, but chances are, you won't be able to tip your campaign. Most of the people raking in 4 figures daily with TS are doing paid traffic via Facebook Ads.

I should know because I was raking in 1-3k per day way back in March. I had like 30-40 campaigns running on FB Ads daily that time. That was also when people were still abusing custom audiences (scraped) from FB Graph Search. My glorious days lasted for only like a month and half. Now I'm down to a measly $20-30 per day from PPI lol. Currently trying several things at a time: PPI and Kindle while still testing stuff on FB Ads. If things work well, I'd be more than happy even for just $4-5k per month.

That's a pretty good run for independent t-shirt sales. With that many leads, I wonder if you could have prolonged the income by capturing their email addresses via a landing page of your own prior to purchasing via t-spring.

Possibly, but not on this particular niche I sold shirts on as this isn't a profession/passion/interest niche. I still have them on my pages though. A few months ago, FB modified their ToS to no longer allow running campaigns like this. Otherwise, I would've re-run them and scaled it up even further.

The t-shirt it's very cool you could sell that a lot with the right strategy. The best use I think would be to use Facebook Ads. You can target on occupation "Doctor", not married, male, under 30 years old, US only. It could work, after all I think you should test it $10 only.

Lordaeron is right. Who is actually going to buy this? You need to find something someone is passionate about and then target the product to their passion. People are passionate about things like Fishing, Shooting or Football. There are lots of things like that that you can target a message that they will wear on the t-shirt. Are there any passionate Doctors that are going to buy this? No. Your message needs to be written by a native English writer too as it doesn't really come across well.

Your not really designing your making fashion/political slogans and putting them on a T-Shirt, so you need to make them poignant and fashionable!

That means hundreds of designs based on what is currently going on. Look in the newspaper. You will never make money with 1 t-shirt and specifically that 1 slogan.

You need to be a good copy writer and create new designs every day.
