Is there any way to do multiple double meta refreshes
for different CPA offers instead of registering a new
domain for each campaign using just one domain.

Yes... just make one at yourdomain.com/1.php, yourdomain.com/2.php, etc.

I used a software program that made 3 files. Should I just put in a index.html in the root and then use the program for each new cpa offer & make a new folder

    1. index.php

    2. links.txt (affiliate links)

    3. second.php (checks to make sure that the referrer is blank)

You could also create various subdomains in addition to pages

Which would be better subdomains or just a new folder & why would it be better.

It wouldn't matter if the redirect is penis.whatever.com or whatever.com/penis - It's pretty much the exact same thing.

Exactly--though subdomains do look a bit more attractive to click on.
